Modifier invalid on the date of service

The modifier on the line was not a valid modifier on the date of service.

CO = Contractual Obligation. The provider owes the difference. The patient cannot be billed for this amount. The same reason code can appear with a different group code; the group decides who may be billed.

Usual causes

  • Retired modifier or one that was replaced (for example payer-specific telehealth modifiers that changed).
  • Typo in the modifier field.
