Recognition is maintained, not achieved once. Every year the program asks the practice to show that the medical home workflows are still running and that quality measures are still tracked and acted on. Practices that treat this as a project each year lose weeks to it; practices that fold it into normal operations finish it in a few hours of staff time.
The PCMH Annual Review is that few hours, organized by Revelrex. We know which evidence the program expects, we pull it from your EMR and your policies, we check that what the documents say still matches what the team does, and we support the submission and any reviewer questions.
Practices that benefit
- Primary care, family medicine, internal medicine and pediatric practices that already hold recognition.
- Practices whose recognition champion has left or changed roles.
- Groups with several recognized sites that want one consistent annual process.
- Practices that were recognized with Revelrex and want the same team for each cycle.
What usually goes wrong
The annual deadline arrives unnoticed
Reporting windows are easy to miss when nobody owns the calendar, and a missed window puts recognition at risk.
Evidence has drifted
Reports changed after an EMR upgrade, a policy was rewritten, a workflow quietly stopped. The annual review is where that surfaces, ideally before the reviewer sees it.
Measures tracked but not acted on
The program expects improvement activity, not only numbers. Practices often have the data and no record of what they did with it.
Staff turnover
The person who assembled last year's report is gone and the evidence library went with them.
The actual work
Reporting calendar
We confirm your reporting window and set the working dates so nothing is assembled at the last minute.
Evidence assembly
Reports, policies and screenshots collected from your EMR and files, named and dated the way the program expects, mapped to each requirement.
Workflow confirmation
Short conversations with the front desk, care team and quality lead to confirm the workflows still run as documented, with fixes where they do not.
Quality measure review
Measures, results and the improvement actions taken, written up in the form the program expects.
Submission support
Entry into the program platform, review with your champion and responses to reviewer questions.

Frequently asked
How long does the annual review take?
Three to six weeks from the first working session to submission, depending on how current the evidence is.
Can you do it if Revelrex did not handle our original recognition?
Yes. We start from your last submission and your current workflows; nothing about the process depends on who did the first recognition.
What if a workflow has stopped?
We flag it, help you restart it, and where the program allows, document the correction. Where a gap cannot be closed in time, we advise on the honest way to report it.
