How we calculate this
Every line uses your inputs times a rate from published industry ranges. Rates are shown so you can judge them.
- Estimated monthly collections = visits per month × average payment per visit.
- Unbilled encounters = collections × 1.5%. Visits that never became a claim: missed charges, lost superbills, encounters closed late. Published range 1 to 3 percent.
- Denials never reworked = insured collections × denial rate × 60% never appealed × 55% that would have been paid. Self-pay visits are left out. When you do not know your denial rate we use the specialty benchmark, raised by up to 3 points for a heavy Medicaid mix.
- Underpayments and uncollected balances = commercial collections × 3% paid below contract, plus Medicare and Medicaid collections × 0.5%, plus self-pay collections × 20% never collected.
- Slow AR carrying cost = for every day above 35 days in AR, 0.1% of monthly collections becomes hard to collect, plus the cost of money tied up in receivables at 8% a year.
- Annual leak = monthly leak × 12. The bars compare the four buckets with each other.
Sources for the ranges: industry surveys on charge capture, denial appeal rates and days in AR published by medical group associations and revenue cycle vendors. Your own remittance data will always beat a benchmark; that is what the free denial autopsy is for.
