Usual causes
- Two visits or procedures on the same day from the same group.
- Second claim for the same session sent separately.
- Distinct service without a modifier showing it was separate.
The fix
Check what was paid for that date. If this service was separate and distinct, add the right modifier and resubmit. If both services belong on one claim, send a corrected claim for the original with both lines.
